GROUP LIFE AND AD&D 

• Guaranteed Issue - 10 to 24 lives, $100,000. Guaranteed issue 24 plus lives - case-by-case basis, but can go as high as $1,000,000.

• Reduction and Termination - 35% at age 65, 50% at age 70, termination at age 75 or retirement. Other reductions available upon request.

• Participation - minimum 75% on contributory cases, 100% on noncontributory cases.

• Rate Guarantees - 1 year and 2 years.

• Accelerated Death Benefit - available. (Not available in all states.)

• Dependent Life - several benefit options available. (Flexible to design-specific benefit needs.)

• Total Disability Benefit - waiver of premium is provided for employees under age 60.

• Conversion - conversion of group life to a permanent whole life policy is available upon benefit termination.

• Eligibility - standard 30 hours per week; 20 hours per week available upon request.
